Cityblock and Centene collaborate to provide primary care services in Florida

Cityblock, a Medicaid value-based primary care company, has joined forces with Sunshine Health, a Centene subsidiary, to provide essential care services to high-need Medicaid members in 11 Central Florida counties. This partnership, which kicked off this month, aims to offer comprehensive care coordination services to members, allowing them to access their multidisciplinary care team virtually, at home, or at a local clinic 24/7.

Cityblock’s care model is specifically tailored for individuals with complex needs who are at a higher risk for poor health outcomes. By reducing unnecessary emergency department visits and avoidable inpatient admissions, the care team strives to enhance the quality of care and member engagement. The company’s 2024 Equity in Action Report has demonstrated increased engagement, clinical continuity, and significant reductions in avoidable inpatient admissions among these populations.

The partnership with Sunshine Health marks Cityblock’s expansion into the Florida market, building on its existing relationships with Centene in New York and Ohio. Dr. Toyin Ajayi, co-founder and CEO of Cityblock, expressed excitement about the collaboration, emphasizing the company’s growth and commitment to providing high-quality care to underserved communities.
