Golden Valley Health Centers introduces innovative ‘Enhanced Care Management’ program

Golden Valley Health Centers in California has recently launched its Enhanced Care Management program in Merced and Stanislaus counties to help patients with complex needs live healthier lives. This program, funded by a grant from the California Department of Health Care Services in partnership with Central California Alliance for Health, aims to provide extra care and support to select members of the community.

Through this program, Golden Valley Health Centers has been able to assist patients in accessing resources such as housing assistance, food assistance, and transportation to medical appointments. One success story shared by GVHC vice-president Amy Collier Carroll highlighted how the program helped a patient and their family secure housing assistance and transportation for medical appointments.

With about 5,000 members identified as qualifying for the program, Golden Valley Health Centers has already enrolled nearly 100 individuals. The lead care managers work closely with patients to address both clinical and non-clinical needs, ultimately helping them avoid more costly care in the future.

“We want to ensure that the people in our communities have access to what they need,” Carroll emphasized.

Golden Valley Health Centers serves approximately 150,000 patients, many of whom are underserved. The organization offers primary and dental care, as well as a range of specialty services to meet the diverse needs of the community.
