Improving Male Fertility: Effective Strategies for Managing High DNA Fragmentation in Sperm

Male infertility is a common issue that affects many couples trying to conceive. One of the factors that can contribute to male infertility is high DNA fragmentation in sperm cells. DNA fragmentation refers to the presence of fragmented DNA within sperm cells, which can hinder the ability of sperm to fertilize eggs effectively, leading to infertility, poor-quality embryos, and an increased risk of miscarriage.

Dr. Sneha Sathe, a Fertility Consultant at Nova IVF Fertility in Chembur, explained in an interview with HT Lifestyle that the DNA Fragmentation Index (DFI) is a specialized test that measures the extent of DNA fragmentation in sperm. While the DFI test is not routinely recommended for all couples experiencing fertility issues, it is advised in certain situations such as long-standing infertility, multiple failed intrauterine insemination (IUI) cycles, previous IVF cycles with poor embryo quality, or low blastulation rates.

Several factors can contribute to high DFI, including infections, environmental pollution, lifestyle choices such as smoking, alcohol, and drug use, as well as medical conditions like diabetes and obesity. High DFI can negatively impact the outcomes of in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) cycles, increasing the risk of producing poor-quality embryos or embryos that do not reach the blastocyst stage, a critical step toward achieving pregnancy.

To manage high DFI, Dr. Sneha Sathe recommended that male partners with high levels of DNA fragmentation in sperm take antioxidant supplements and make lifestyle changes, including diet modification, regular exercise, and stress management through activities like yoga and meditation. For couples where the male partner has high sperm DFI, IUI cycles are generally not recommended, and IVF with ICSI is suggested instead.

During an ICSI cycle, techniques such as the Zymot Slide, PICSI, or MACS may be used to obtain sperm with lower DNA fragmentation. Additionally, men with high DFI are advised to have shorter periods of abstinence before providing their semen sample, as this can reduce the DFI on the day of sample collection.

Dr. Sneha Sathe emphasized that with proper management and advanced techniques, couples can improve their chances of successful conception despite high DFI. Consulting a fertility specialist for tailored guidance and timely intervention is crucial in overcoming infertility challenges and achieving successful pregnancy outcomes.
